I think, there is some fundamental problem with your tex4ht
installation. I tried a file called, test.tex, I could not replicate
your problem. The log in my case for the corresponding location looked

 Execute script `test.lg'
 t4ht.c (2010-12-16-08:47 kpathsea)
 t4ht -f/test
 Entering test.lg
 Entering test.css
 Entering test.tmp
 System call: cp test.xml test.tmp
 System return: 0
 System call: java -classpath /opt/texlive/2011/texmf-dist/tex4ht
  /bin/tex4ht.jar xtpipes -i /opt/texlive/2011/texmf-dist/tex4ht
  /xtpipes/ -o test.xml test.tmp
 System return: 0

Anyway, I am attaching an archive (test.zip) of all files of my test
run which might be of help to you. You may kindly do the same and send
me the archive off the list.

Best regards
